Child Application Form

You will be asked to complete this application form if your child has been accepted for a part in our pantomime or any other production this year and they are in school Year 5 or above and under 18. We are really looking forward to working with you.

Please ensure that your child will be available for all dress rehearsals and performances before accepting a part. The rehearsal details are posted on our website and/or our Facebook group.

Young people are expected to:

• Check the Rehearsal Schedule and attend rehearsals on one evening a week when required. Principal characters will also be required to attend some extra rehearsals. At some point, like after Christmas for the panto, everyone will be needed twice a week. As we near the performances, some rehearsals will run until 10:00pm – you will be told when this will happen. If you are unable to attend a rehearsal for some reason, please let the Director know in advance.
• Show respect for all members of the society and behave as asked. The Director has the right to contact parents if a child’s behaviour is considered unacceptable. This may result in the child no longer being a member of the company.

FDS members are expected to:

• Work in accordance with the Child Protection Policy.
• Take appropriate steps to ensure the safety of all children at all times.

All parents/guardians are expected to:

• Ensure their children do not arrive too early for rehearsals, as there will be no adequate adult supervision before the appointed time.
• Collect (or arrange collection of) their children after rehearsals or performances. It is NOT the responsibility of the society to take children home.
• Access and read the FDS Child Protection Policy and Privacy Policy via our website.
• Allow use of photography and/or videos as part of the production.
• Allow your child to be filmed or photographed during the activity, with the possibility that these photographs/media recordings may be used for social media and marketing publicity. FDS will take all steps to ensure these images are used solely for the purposes for which they are intended.

Some parents may be asked to volunteer as chaperone for the children during a rehearsal or performance. Further details downloadable here – FDS Child Protection Procedures. The Pantomime is a huge commitment by FDS both in staging the performances and in taking proper care of the young people who come and take part with us. We would like to thank you for all the support that you give as we could not do any of this without it.

Payment of Fees:

Members: If you are a current member of the society with a child performing in a production then no payment is required but we do need you to complete this application form for each child if not already done recently.

Non-Members: To help towards our costs, and for insurance purposes, we ask for a payment of £10 per child* if you are not already a member of the society. Your child will then remain a member and be covered for any other productions for the year. If you have 2 or more children you might consider joining as an adult member, which will include your children.

Payments are made using our secure payment gateway, either by using your debit or credit card or through a PayPal account (if you have one).

* Please do not pay any money until your child has been allocated a part as all payments include a small but non-refundable transaction fee.

Financial Statement: All money received from membership fees, ticket and bar sales, and the occasional donation is used to cover the running costs of our productions and for our charitable pursuits. The greatest expenditure is for venue hire for both rehearsals and performances, copyright licensing, and insurance. Other expenses include staging, lighting, publicity, costumes, props, and storage.
